On this episode of Advances in Care , host Erin Welsh and Dr. Craig Smith, Chair of the Department of Surgery and Surgeon-in-Chief at NewYork-Presbyterian and Columbia discuss the highlights of Dr. Smith’s 40+ year career as a cardiac surgeon and how the culture of Columbia has been a catalyst for innovation in cardiac care. Dr. Smith describes the excitement of helping to pioneer the institution’s heart transplant program in the 1980s, when it was just one of only three hospitals in the country practicing heart transplantation. Dr. Smith also explains how a unique collaboration with Columbia’s cardiology team led to the first of several groundbreaking trials, called PARTNER (Placement of AoRTic TraNscatheteR Valve), which paved the way for a monumental treatment for aortic stenosis — the most common heart valve disease that is lethal if left untreated. During the trial, Dr. Smith worked closely with Dr. Martin B. Leon, Professor of Medicine at Columbia University Irving Medical Center and Chief Innovation Officer and the Director of the Cardiovascular Data Science Center for the Division of Cardiology. Their findings elevated TAVR, or transcatheter aortic valve replacement, to eventually become the gold-standard for aortic stenosis patients at all levels of illness severity and surgical risk. Today, an experienced team of specialists at Columbia treat TAVR patients with a combination of advancements including advanced replacement valve materials, three-dimensional and ECG imaging, and a personalized approach to cardiac care. Finally, Dr. Smith shares his thoughts on new frontiers of cardiac surgery, like the challenge of repairing the mitral and tricuspid valves, and the promising application of robotic surgery for complex, high-risk operations. He reflects on life after he retires from operating, and shares his observations of how NewYork-Presbyterian and Columbia have evolved in the decades since he began his residency. In the Drop Your Buffs Survivor 47 finale recap, Sean and Evan celebrated Rachel's win while critiquing the predictable format, the fire-making challenge, and Sue’s underwhelming age reveal. They lamented the absence of early boots like John Lovett from the finale and called for changes in casting and editing to revive Survivor's character-driven magic. The duo praised Rachel's gameplay but highlighted how modern Survivor's strategy-heavy edit leaves winners less memorable. Speculation on Jeff Probst's scent (Caesar salad?) and a call for the wind-and-fire emoji in honor of Teeny’s fire-making struggle kept the recap lively. Episode recap starts at 14:15 after our general chat! This week on Drop Your Buffs recap for Survivor 47: Bob and Weave, Sean and Evan delve into the penultimate episode’s biggest moments, including Rachel’s game-saving idol play and Andy’s unraveling post-Operation: Italy. They discuss the dynamics of Genevieve and Rachel’s ‘rivalry,’ Teeny’s chaotic self-awareness arc, and Sue’s miscalculated(?) idol play that left fans divided. The hosts analyze the evolving storytelling of modern Survivor, Jeff Probst’s hand in the narrative, and whether this season delivered on gameplay or fell flat on strategy. With Star vs. Starless coming up on the Aftershow, a toast to drunk Teenie, and fan questions in the mix, they sign off with their sights set on the upcoming finale. This week Sean and Evan are discussing "the best episode of the New Era." We're talking the overnight reward, Italian feast, a basil bouquet, the classically Italian margarita, Operation: Italy, Andy's powers of persuasion, Rachel getting duped and confessional prowess. Plus, we work very hard to make sense of Evan's notes on the episode including Caroline's potential as a And Just Like That ancillary character. This week on Drop Your Buffs, Sean and Evan recap Survivor 47: Flipping the Windswitch and dive into standout moments from episode 11. They explore the dynamics of Sue’s mysterious dirt patterns, Genevieve’s evolving gameplay, and the rise of Rachel as a central figure in the season. The hosts dissect the formation of final three alliances and ponder how certain players, like Teeny and Andy, are positioning themselves for the endgame. The duo also breaks down Rachel’s journey challenge, discussing its design and its role in the narrative. Caroline’s memorable Tribal Council comment sparks a conversation about her enigmatic presence this season. They also critique the editing and storytelling, questioning the lack of personal moments that give depth to the cast. The episode starts with a discussion about Jeff Probst’s TikTok song, the cultural significance of American Thanksgiving, and the intersection of global pop culture and fame.
